Pain or unusual sensations in the arms and legs can have many causes, and most are not due to a heart attack. However, certain symptoms—especially when they occur suddenly or alongside chest discomfort, shortness of breath, nausea, or sweating—can be warning signs of a serious cardiovascular problem.
1. Pain or Discomfort in One or Both Arms
A heart attack can cause pain that:
Starts in the chest and radiates into the left arm
Occurs in the right arm, both arms, or shoulders
Feels like pressure, squeezing, aching, heaviness, or burning
This is one of the more classic heart attack symptoms.
Relevant condition:
Myocardial Infarction
2. Sudden Arm Weakness
Sudden weakness in an arm is more commonly associated with a stroke than a heart attack, but it can signal a serious medical emergency.
Relevant condition:
Stroke
Seek immediate medical attention if arm weakness occurs with:
Facial drooping
Difficulty speaking
Loss of coordination
3. Numbness or Tingling in an Arm
Numbness can result from nerve compression, but sudden unexplained numbness—particularly when accompanied by chest pain, shortness of breath, or sweating—should be evaluated urgently.
4. Pain in the Shoulder, Neck, or Jaw Extending Into the Arm
Some heart attacks present as:
Shoulder pain
Neck discomfort
Jaw pain
The discomfort may spread into one or both arms and can occur even without severe chest pain.
5. Leg Pain While Walking That Improves With Rest
This is not usually a sign of an ongoing heart attack, but it can indicate narrowed arteries in the legs.
Relevant condition:
Peripheral Artery Disease
Symptoms may include:
Cramping in the calves, thighs, or hips during walking
Relief after resting
People with peripheral artery disease often have a higher risk of heart attack and stroke.
6. Swelling in the Legs, Ankles, or Feet
Persistent swelling can be a sign that the heart is not pumping blood effectively.
Relevant condition:
Heart Failure
Other symptoms may include:
Shortness of breath
Fatigue
Rapid weight gain from fluid retention
7. Cold, Pale, or Bluish Arms or Legs
Poor circulation can sometimes cause:
Unusually cold limbs
Pale skin
Bluish discoloration
These symptoms may indicate reduced blood flow and warrant medical evaluation, especially if they appear suddenly.
Other Heart Attack Symptoms to Watch For
A heart attack may also cause:
Chest pressure, tightness, or pain
Shortness of breath
Cold sweats
Nausea or vomiting
Lightheadedness
Unusual fatigue
Women, older adults, and people with diabetes may experience less typical symptoms.
When to Seek Emergency Help
Call emergency services immediately if you have:
Chest pain or pressure lasting more than a few minutes
Pain spreading to the arms, shoulder, neck, jaw, or back
Shortness of breath
Sudden sweating, nausea, or dizziness
These symptoms can indicate a heart attack and should not be ignored.
Important Note
Symptoms in the legs are more often signs of circulation problems or heart failure rather than a heart attack itself. Arm pain, especially when combined with chest discomfort or shortness of breath, is more directly associated with heart attacks.
